Star Eon Homecare LLC is a non-medical in-home care agency. We provide personal care, companionship, supervision, and assistance with activities of daily living. We do not provide medical or skilled nursing services. Even so, we may receive or create health-related information about you while providing our services, and we are legally required to protect it.

Billing and Payment

  • Processing payments, invoices, or reimbursement

  • Verifying coverage or eligibility, if applicable

Business Operations

  • Quality assurance and internal reviews

  • Training, supervision, and administrative functions

  • Compliance, audits, and licensing requirements

Legal and Regulatory Requirements

  • When required by federal, state, or local law

  • In response to lawful requests from government agencies

Mandatory Reporting (California Law)

  • Reporting suspected abuse, neglect, or exploitation of elders or dependent adults, as required by California law

We will limit uses and disclosures to the minimum necessary to accomplish the purpose.

We will not use or disclose your PHI for the following purposes unless you give us written authorization:

You may revoke your authorization at any time in writing. Revocation will not apply to actions already taken.

You may request to see or receive copies of your PHI in paper or electronic form.

If you believe information is incorrect or incomplete, you may request an amendment.

Right to Request Restrictions

You may ask us to limit how we use or share your information. We are not required to agree, except where required by law.

Right to Confidential Communications

You may request that we contact you in a specific way or at a specific location (for example, by phone only or at a different address).

Right to an Accounting of Disclosures

You may request a list of certain disclosures we have made of your PHI.

Right to Receive a Paper Copy

You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.

Right to Be Notified of a Breach

You have the right to be notified if a breach occurs that affects the privacy or security of your information.


Our Responsibilities

We are responsible for:

  • Maintaining appropriate administrative, physical, and electronic safeguards

  • Using and disclosing PHI only as permitted by law

  • Limiting access to authorized personnel

  • Following the terms of this Notice

  • Providing breach notifications when required


Complaints

If you believe your privacy rights have been violated, you may:
